Nah, he probably just doesn't technically meet the requirements. Go hand some of the items to the Guide NPC to check you have everything, and make sure you stand next to one of the weird, glowing, unbreakable red/purple things in the crimson/corruption.

Hm. And you're standing next to the altar? I've never heard of an item refusing to craft, but it's very easy to mess up and not be standing in quite the right place. Are you running any mods?
