The grappling hook isn't working now.

I don't know why, but it seems my copy is somehow EXTRA buggy.
First I had the weird thing with being forced to click the first option that comes, but I sort of learned how to deal with it. Now we have another issue:
Earlier today I made a grappling hook which proved to be very handy. I spent some time being spiderman through unfriendly terrain, got back to base (via death), saved and closed the game. Now the grappling hook doesn't work. It's there, it's in the inventory, but when I leftclick or even rightclick, it doesn't do anything.

E is bound to Grapple.

grappling hook is not binded to any mouse buttons by default

You have a special inv slot for it, just right click the item in your inv and you now have more space.
