Yes, the worm scarf takes off 15% damage from all sources. If you take it the ironskin and endurance potions, you can become about 50% resistant to all damage types.
although it also depends on the class which i take you are a mage class right?
you can also afford to lose the emblem as you have a mana flower, which instantly consumes mana potions, so you can afford to keep on continous damage.
you can swap out most of your items, except probably wings and the ankh shield for sure. (the wings swap out if you get an infinte flying mount like the flying saucer or the pigron mount)
the only way i'd say you can swap the worm scarf is if you get the healing stone from the golem, which helps you regenearte alot faster (best keep it still in the accessory vanity slow so you can heal, then go back to defense)
Mages are extra squishy, especially in Expert, so I'm not even sure how you're surviving with no Warding accessories :O
In fact, how are you surviving without a Cross Necklace? :O
Sheesh, if you're going all-out damage then you may as well chuck away the Worm Scarf...
... each to their own, and that's also lategame.
