There are a few changes here and there like trade route trains are now dangerous if you are standing on the tracks, the Fort build option is completely redone ( I don't really care for it, but its minor compared to the GOOD parts of the mod), Walls have more upgrades and can now be built over trade routes. Buildings (except town cernters) are rotatable, There is a new resource, "Fame" that is totally optional. It is used to order unique units.
The difficulty seems relatively the same. I'd check it out at least, from the link above.
