ETF unlocks minor and emergent factions (emergent factions are factions which can appear during the game such as Quebec, the United States and Ireland.) It does not make many changes to vanilla game-play. (It does allow Fifth Rates to be built at any port, which is a bit odd. However, it gives smaller factions a chance to avoid losing our trade income because of a sea blockade.) I love using ETF, because I enjoy the challenge of playing as smaller factions and the variety of starting positions.
Another option for unlocking minor factions would be to just use the 'all minor factions unlocked' startpos: http://www.twcenter.net/forums/showthread.php?262048-All-Factions-Unlocked-No-FOW-Bug!-WORKING!-Downloadable-Now!&p=15094003&viewfull=1#post15094003. This allows you to play minor factions (such as Denmark, Venice and Georgia) but not emergent factions.
Not all factions which are unlocked are really playable, however. The Pirates can only recruit six sword units (you can play the Pirates using other mods such as Minor Factions Revenge or Pirates Uber Alles). Some factions have very limited unit rosters; you can expand their rosters using a mod such as the Additional Units Mod or the Minor and Emergent Factions Unit Pack, which you can find here: http://www.twcenter.net/forums/forumdisplay.php?1520-Unit-Packs-and-Stats. Some specific faction packs are available, for example I had a lot of fun playing Venice after I added Pom's Italian States Mod in this campaign: http://www.twcenter.net/forums/showthread.php?660374-The-Lion-and-the-Eagle-(Venice-AAR).
Many of the larger, hosted mods such as Imperial Splendour, Imperial Destroyer, Empire Realism and DarthMod unlock extra factiions (usually minor factions).
