Can't seem to pick-up attack Mod in treasure room?
It just blinks some empty slots below my gun icon, and does nothing.

I think i figured it out:

1) the first time i came in this room i had 0 mod slots.

2) the 2nd time (in my screenshot), i have 1 empty slot & 1 filled slot, so i can't equip the 2 star/slot mod.

...i think?

yeah, that;s right.
each gun has it's own slots ( some have none ) and when theyre filled, you can't replace them with new ones - you need a new gun

You can replace mods on your currently equipped gun (or at least you can when buying mods). In this case you already have a banishing mod on your gun I think, therefore you can't equip a second instance of the same mod.
Stars just indicate how rare an item is. Mods always take one slot no matter their star rating.

you can replace mods actually. When you stand close to a mod, you can see some of your own mods in the bottom left corner flash in a certain order. The mod highlighted is gonna be replaced with the new mod you want when u take it. Be careful its a bit fast

That's my issue though. In the picture i have an open slot, and i still cannot equip the mod.

EDIT: Oooooooooh! Nvm, its the SAME mod. That's why i can't equip it.
Just some really bad luck for a first run, i guess.

The starting gun should really come with 1 free mod slot, since it gets replaced immediately, and lone-slots are hard to come by anyway.
