Report a translation problem
If it's still bugged after all this time then that's very disappointing. I've had this issue since a few patches after the 1.0 launch. T.T
1. I Did to ensure it wasn't bugged.
2. This shouldn't be the case, but i do them anyway because it makes the game easier
3. I'm not sure if they count, but i did them anyway
4. I am not sure, i did it on a normal run.
I was going crazy with this achievement in particular, and i believe it is because sometimes TWO red portals can spawn on a level... And missing them is quite easy.
My suggestion is that you pick the item that shows you if you're closeby to a portal, item or hp machine.

Tbh, I had this bug since the update that it was introduced so I think my save is just cooked and I don't have the heart to start from scratch. I've definitely used the item seeker, and am getting to the point where I know where all of the items/portals spawn on each individual chunk of level generation XD
Maybe if #1 wasn't the case, it would be more manageable because I could actually get cores instead of the RNG trash item that ruins your build, but even then, there are only so many times I can get the achievement without it popping before I go insane.
