99 are obtained by writing a history with a numen, being every combination of aspect and numen, with some extra variations for matching your origin with the right numen.
The last two are either not doing that and just presenting the journal... or doing something very particular. If you want to get spoiled, the wiki has a list of them all.
There are 13 Numens (Numina?), and each one has 3 Aspects. Each Numen-Aspect combo yields a unique ending, for 39 main endings. Each of these endings also has 1-2 variants for specific Librarian origins (the Numen will say which origins want it), which are the same general events with a slight twist. And then as Stim said, there are 2 other endings that have nothing to do with the Numen system. You can position a single save file to hit all of the Numen-Aspect combos in rapid succession if you're willing to reload, but to get the origin-specific variants, you'll obviously have to do 9 playthroughs, one per origin.
