Yes, you can upgrade a Soul card to ++. You can also upgrade it to +++.
2. Skills have 2 schools before you commit them, which means 2 choices for what soul you can get, if you don't like the choice for one school for a given skill, pick the other one.

There are four levels total (starting, +, ++, and +++).
There's a misconception here. Illumination isn't tied to Phost, your skill is tied to Illumination and your choice of using that skill for Illumination gave you Phost. Illumination skills can also give Mettle in which the correct skill with Mettle would fit there. Just like Illumination isn't tied to Phost, Phost is also not tied to Illumination. You can also get it through Ithastry depending on your skill you slot in Ithastry.
I answered half above. The way you determine what Soul you're going to get is to look in the top right corner of your screen when the skill is slotted but not committed.
Yes, you can obtain ++ level cards. I've even heard theoretical discussions about +++ cards but I have yet to see anyone post pictures of those.
I'm not going to boot up BoH just to take a pic, but I have a +++ in Health right now, so I can confirm it's possible. It has 3 Heart / 3 Nectar / 2 Scale.
Good to know! I've just seen people talking about how it's theoretically possible and no one actually saying that they had done it. Considering it takes 8 souls it's quite an accomplishment.
