Her dilemma is that has nowhere to live if she quits her permanent job at the hospital and goes part-time.
They give her a slightly gritty past - her alcoholic, gambling-addicted father died in a pool of his own blood and she found her mother hanged in the local park a few years later
Her sexual harassment/"I want a girlfrieeeeend" style is completely turned on its head when you try to get close to her. If you're not on track for her official Bad Ending, she'll reject you like "falling for me is what makes you a dummy" or "you're so pure, Kaori, you deserve better than a crappy person like me"; but as you get closer to her Bad Ending she becomes more cruel and I personally found her completely unlikeable as a character by the time I was done.
The CG you might have seen where Kaori's holding Yamanouchi's foot? It turns out to be nothing to do with her healing hands, rather Yamanouchi laughs in your face when you tell her you love her and tells you to prove it by kissing her feet.
Yamanouchi is openly uncapable/unwilling of trusting anyone and because of Kaori's simp nature the bad ending ends with you in a Yamanouchi-based personality cult/abusive relationship, at work as well as in your personal life. What I found slightly heartbreaking is that Nagisa and the Chief ask Kaori what's going on because they're concerned about her strange behaviour, and the Chief actually appears horrified when Yamanouchi refers to Kaori as her "darlin' meat doll"..
The game ends when you go back to Yamanouchi's that night for your slave duties. In this route of the game, you never actually get to call her "Yasuko".
In the final scene, Kaori wears bondage gear, which is a redeeming feature, I guess, if you like that sort of thing?
