Did I miss something or is this backwards?
Deathpay is paid at hiring, not on death... do I get it back if the crew NPC is fired before dying? Or is it a hazard bonus?

Yes, you get it back when ending a contract, not in death.

Yes! "death pay" is basically like a crude form of insurance/deposit on someone's life.
you pay upfront and are incentivised to keep them alive so you can reclaim your money.
if they die you cant get it back.

What if they die before we pay it to them? I have a late fee but they are dead. Do I still need to pay it to keep from incurring more fees?

Yes, because it's an insurance/life policy you owe it to the crewmember's 87 space cats, or whoever they willed it to.
