You have to restart the game for the change to take effect.
That soundfont uses the Roland instruments you are likely familiar with. It comes bundled with a program called Synthfont, so if you have trouble finding just the soundfont by itself you can always install that and get it that way.
If you want something a little fancier, you can download this Roland SC-55 soundfont[dosboxece.yesterplay.net] found on the DOSBox ECE homepage.[yesterplay.net] It uses recorded samples from a real Roland SC-55, which sounds a little better than the raw instruments on their own. You have to extract this before you use it of course, and I'd rename this to sc55.sf2 or something before you use it because the filename has spaces (won't work in the console) and the extension is duplicated for some reason. You can use filenames with spaces in the console as long as you encapsulate the filename in quotations, if you really want to.

Make sure you hit Enter to actually send the command. You can actually copy this line right from this post and use ctrl-v to paste it into the console so you don't have to type it out. Now just restart your game for the change to take effect.

This doesn't do anything to fix the audio balance, it just changes the instrument sounds to be the ones you are used to if you played the game with a Roland Sound Canvas back in the DOS days, or if you played it in Windows/DOSBox in which case it used the Microsoft MIDI synth which uses the same instruments. These are the same instruments that were likely used to compose the music as many of the games of the era were composed on hardware that used these same instruments; Doom and Duke Nukem 3D for instance. My suggestion regarding the volume is to simply turn the music volume all the way up in the game options, and turn the sound effects down to a comfortable level. Music 100%, sound effects 50% works well enough for me.
If you ever want to go back to the instruments the game ships with, just use this command:

Ah, Thanks a million for the very in depth instructions! Super helpful, it sounds more faithful now. For me, I have to turn sound effects all the way down to 30% as Caleb's voice is screechingly loud at 50% and above for me(weirdly, CD music's volume is fine, its the Midi music that is awfully quiet.) Really sucks to see Atari abandoning support for the game, still so many simple issues like these no longer have any hopes of getting patched. Thanks again, cheers.
I would only recommend "Blood GDX" anyway because its by far a better source port from the Original Blood You can use this with "Fresh Supply"

Edit: Oh I see you don't even own Fresh Supply..
BloodGDX is obsolete now thanks to Nblood, it still uses Java and doesn't have proper colours + inaccurate physics.
Not on Steam now.
I bought it on Steam release and then asked for a refund.
