CSD3 is a little more story-based than the previous two games, so the endgame is just the final area of the game: the Iron Cook Speedway. It's harder levels with some unique rules and content, but it's still mostly the same as the rest. There isn't anything new post-game. There's nothing specific to do after getting all gold medals unless you just want to play more. (Although there are some achievements that require you to get gold medals with more specific requirements above-and-beyond a normal one).
It took me about 65 hours to get all golds and beat the game. For what it's worth, I think this is the best game in the trilogy and I'm very happy with it.
