Right now, you can get used copies in good condition for about 3000￥. Amazon doesn't ship overseas though, so you need special proxy services. I use http://goody-japan.com/index.html myself - they aren't the cheapest, but are very simple to use and reliable.
As for locale - yes you need JP locale to install and run it. Audio was never translated, so it's JP in Steam version as well (some voices were altered due to censorship though).

We talked before, but where would you advise to get the Japanse version of If My Heart Had Wings? And, do I need to set my PC to Japanese locale for it to run? The game features Japanese audio, correct?
