every brave new world one short?

The "where no one has gone before" missions - go to planet, find 4 things; all but one so far does not has the last item. I beam down, inspect one, told find X more; I crisscross, pattern seach for hours, and always it's X-1...
Any insights? Anyone else having this problem?
The best part of the game for me is finding new worlds; I'd like to get full credit for finding and exploring them as well.

Well, many hours later and I now have a collection of pix of shipmates in a corner facing the wall refusing to come out; beamed down crews 5 feet underground, and what is with all the server errors? Everytime i fly into one of those red cloudy things, the server errors out and we are auto-warped out and back in to repeat it all over again, losing credit for all kills in the crashed session.
Still, it's FREE!
