I am not entirely sure but it seems like it happens when Phos is there. At first I thought the elevator was causing the issue, but it happened aswell during the triple fight. I beat him twice without having any damage indicator, unless maybe the small fire damage does not show up (I did not see any health reduction on the fight) Also, it is very akward to do those achievements, if you kill the boss and do not get it, well you would have to shut down the game to reset the checkpoint. It's intentional that I can't move using the left stick of the gamepad? Only the D-pad works... o.o I have tried with a Steam Controller and a generic one, it can't be changed in the options.
