Complete the game after finding all secrets, completing the puzzle, and entering the password into the computer in the final level for the alternate ending

Found on a dead body in Chapter 2

Jones - Alligator - More Gore
Found in chapter 5, collect a crowbar, and before you leave walk around the back of the building and unlock a sewer grate

Carl - Grasshopper - Start With Drill
Found on a body in the disco during Chapter 8

Jake - Snake - Killing Throws
Found on a dead body in chapter 10

Richter - Rat - Start With Silenced Uzi
Found at the top level of the police station in Chapter 13

The masks that are unlocked via high scores just mean you have to beat the preset score that pops up at the end of a level. I'm not all that good and I still managed to get all of them on my first try. You can tell if you don't have one of these "main story" masks by going to chapter select. Under your score, there should be a mask icon and purple tile (or a greyed out mask and grey dot, if you didn't collect it yet).
