Always on top?

For some reason, it won't give me the achievement. I've done all the ground kills at least twice over, but it won't give it to me.

In particular, have you performed an execution using the following weapons:
>>> a) the frying pan that can only be found in chapter 5? <<<
b) the pot of boiling water in chapters 2, 4 and 7 (both empty and full)?
c) the pool cue (both broken and unbroken)?
d) all relevant throwable weapons (dart, scissors, bottle, brick, hammer, shuriken)?
