Weapons Go Missing On Loading Saved Game

Just wondering if anyone else has had an issue with weapons suddenly disappearing without them breaking. Have a weapon when I save, but when I reload the weapon is gone. I repair everything every time I leave the dungeon, so I'm not sure what's happening here. Maybe it's just breaking and I'm not noticing it, but that seems unlikely.

Actually, I have a theory about what's happening, but I'm going to test it out to see if it's repeatable. Will post results if they appear to be helpful.

It's possible that you accidentally switched to your 2nd weapon set, the default to switch back and forth between your weapon sets is the W key. If your weapon broke, it would still be there btw.
