Hey there! If you're having trouble unlocking Insane Mode, it's possible that a level got missed on the map somewhere. This is especially common if you've been playing online with a bunch of different people, as they may have picked a start point beyond what you've done.

Be sure to review your map carefully and make sure that every level got played. Once all your levels are played, then beat the boss. :-D
