Whats the item that is horrible or made your run just CRAP? mine i think lemon mishap it does Very low damage takes a couple of rooms too charge and is just like 3 seconds very bad item and wastes a perfect space for an item

The worst item is the Cursed Skull. There is no item worse than this. At least other items can be used or exploited to be useful, or rerolled. Dead cat is awesome.

How can a damage up with no possible downsides make you lose a run?

I think that lemon mishap isn't THAT bad. Don't understand me wrong here but, if you use it against pin he is instantly dead .That's the only use i can find for it.

Everything freaking Cain run I have had I have always run into the Kamikaze item (probably why I haven't beaten the game yet as him). So I hate that item alot, it's pretty much an insult to go into your first item room and find that..with a character that only has two hearts. I suppose it might be a bigger insult if it was Judas.

Every item is good for something, even Kamikaze (if you keep running at the wall you can use it without getting damage), even lemon mishap can make some damage. But toothpicks is a dead item, it doesn't change your character (other than looks) in any way regardless what it's written on the screen.

Items may be 'good for something', but it doesn't stop them from being on their lists because they're pretty bad at their job. Kamikaze is subpar even with the glitch, and Lemon Mishap has sh­it damage and can't even harm flying enemies and a lot of enemies just phase through it cause they move around.

False. Toothpicks are a 16% shot speed increase.

Either Lemon Mishap or Holy Water. Leaning towards Holy Water since you can use Mishap (nearly) any time and wherever you want. Holy Water, you have to get hit, and even then, it might not even touch the enemy that hit you.

Well by worst item, do you mean most unuseful or most harmful? I'd say wiggle worm is an EXTREMELY unuseful item, and most harmful would have to be the crystal skull
