what do i do after beating the heart?

You beat the Heart 9 times to unlock It Lives. After beating It Lives you can continue on to Sheol/Cathedral. If you're lucky you might get the trapdoor the Sheol spawn before you've unlocked It Lives, but that's quite rare.

Pretty much this. If you haven't defeated the heart 9 times, you just open the big chest.

It was actually like 50% to get a devil/angel room after defeating the heart, so I could get to Sheol like 4 times before It Lives appeared. (in fact I had 3 sheol achievements done by the time I unlocked It Lives!..).

There is a chance you can get a devil room after you beat the heart. Tip: if you want to increase your chance of getting the devil room, take no red(perminate) heart damage and playing as judas( the starting item he has will increase the chance of getting the room by 35%).
