achievement bug?
I recently had an amazing run with cain that led to the final defeat of ???. Afterwards, It said "secret unlocked, cain's eye". When the achievement popped up though, it said ???'s soul is the item I got. Is this a common bug?

It is indeed common for the unlocks to get switched between characters for beating the chest. I had the same as well.

Just beat the chest with every character and eventually you should still be able to unlock everything.
