If you mean monsters respawning and you starting in town whenever you start up the game, that is a feature of ARPGs, not an issue with saving.
I use mods but never had this problem no matter how many different characters i have on the go using a different mod for each one, that was until i installed Forgotten Gods.
It does save, just not all the time, not any more.
1. Mods no like the "Forgotten Gods" and somehow ruin the saving process;
2. Another weird Windows thingy (OneDrive maybe, if you got that one installed);
3. Try run repair.exe in the game folder, just to get that out of way.
I then quit the game and restarted my computer, loaded the game back up and continued my game and it didn't save properly, i still had a full inventory and a quest i finished wasn't.
I have cloud save disabled in steam and in game so i am lost to what's going on.
If you use / run windows 7 than i don't know, but in that case i still would try to run admin mode. Also Antivir / Security Software like GrumpyKitten mentioned can be the fault here as well.
