Forming Italy as the Two Sicilies
1st off, there are a few things you must know. The only nations that can form Italy are
Kingdom of the Two Sicilies, Piedmont-Sardinia and the Papal States,
but it's suggested that you do it with the Kingdom of the Two Sicilies.

Now, the requirments to form Italy is to have all Italian minors under your SoI. As Kingdom of the Two Sicilies, the best route you can go with is to take Tunis, then work on builfing up your millitary power. The Papal states are a good target to pick on after Tunis. Just be awear of their alliances.

After that, your nation should look somthing like the picture to the right. By then, your prestige from gaining Tunis and Papal states, as well as the extra troops you'l earn as a result should make you a great power! Now, focas your influence on taking the minor Italian nations from their master's spheres of influence. This part may be hard, but it's alot easier than trying to annex every minor there.

If you get all of them under your influence, you can finaly form Italy!
Forming Italy as Piedmont-Sardinia
Forming Italy as Piedmont-Sardinia is much easier than forming Italy as the Two Sicilies, thanks to Giuseppe Garibaldi and the event that lets lose the "Expedition of the Thousand" unto all of the Italian States. This event can happen at any time around the game.

Starting position

You could go with the same direction as the Two Sicilies, but you'll have France on your side, with can be a big plus for you. Like the Two Siclies, Tunis is always a option, but you also have Egypt as well as the other North African States to take a bite out of. And don't limit yourself to just un-civs. Austria is always a option, but I would'nt suggest taking Lombardia from them yet, Doing so will result in having you cecced 2 regions, Savory and the providence of Nice to France, in return for some prestige, you gain cores on Lombardia and a increase of 100 relations with France.

Even before the Expedition of the Thousand event happens, you can still go ahead in reaching GP status.

When the Expedition of the Thousand event does happen, all of the other Italian states will experience a large rebel uprising, with the rebels labled "Garibaldi's redshirts". Those rebel bands, ranging from 9,000 to 30,000 ussaly have the job done in more or less than a month, and when all of the Italian nations have fallen under these nations, you will be offered the crown of Italy. Piedmont-Sardinia will have their name changed to Italy, the capital switched to Rome, and of coruse, gain some prestige.
Liberation of the rest of Italy
Now, with your newly formed Italy, you can en-act a desision that will give you cores on South Trol, Venita, Lombardi and parts of Croatra and Slovinia. You'll have to fight Austria for them. A good idea is to form a alliance with the North German Confederation/Prussia and when you attack, call them in. The germans will be able to help with the northern front while you can focas on the Southern front. For a more historical feel, attack Egypt and take Crete from them. Then take on Greece and take the Ionian Islands from then, if possible.
Now, you can be free to expand into Africa or chop away at Austria!

I've been occupying all of the 2 S for 12 years by now, revolt risk is at maximum, but no redshirt has ever shown up.

@dantheman yes you can form italy with tuscany (you might have to reload a few times to get a good alliance). It's a bit more challenging than with S-P and 2Sicilies but i did it and i'm just an average player. Forming India or Yugoslavia are much, much harder.
