Some mild spoilers were unavoidable and while I focused on getting every achievement this guide can help you if you're stuck. Somehow there's a full video of Basement too, it kinda happened.

General tips and info
Okay, this is primarily an achievement guide but it also covers every Digression so if you're stuck on something particular it'll help too. I'll divide Entities\Encounters plus misc achievements (there are some) by the Digression I found them in, due to RNG you'll need to go to every Digression at least twice (much more for achievements but if you're only after those this guide will make it much easier) and you can probably encounter some of the Entities in other places, feel free to add that, RNG hates me with gusto so there's always a possibility that I missed something despite seeing almost everything this game has to offer, either way help would be appreciated.

If you're here before you actually played the game just go and play it, first three Digressions are easy and won't give you trouble. If you're done with Hallways I got just one piece of advice for you: save in separate slots every chance you get and good luck! With the Basement being really, really long you'll need both luck and those saves, otherwise revisiting Digressions is a breeze and you can collect stuff you missed afterwards, after you're done with the Basement you can freely explore and get more stuff plus your stats will be higher which will help. I also suggest you turn on the stat regen in the settings just to make life easier at first, don't do it if you're after a challenge. There are also certain events and Entities you can only see with low stats so when you feel comfortable enough to explore do try to experiment. Stock on items, you'll get enough Credit to get by as you play and those items will help. Also when the game asks you to use something (mainly a Cloth Rag or a Stick) - do it unless you want to try your luck or purposefully lower your stats.
Also obvious spoilers ahead, can't do this without 'em, everything from the Basement and onwards contains serious spoilers and lots of lore references and cross-references. For the sake of clarity I use they\them pronouns for every Entity, just don't want extra confusion. And again for the sake of clarity I dubbed Leisure Centre 'Mall', it just looks like one to me and takes way less space.
Note that most of the extra exploring is done after completing all Digressions, it was easier that way for me but it's also possible I'm missing something because of this, so far I didn't see anything like that but again it IS possible and there are some things you should do before you enter Basement for the first time, they're all listed below.

Now I use underlined text for Entities you can Encounter, bold for achievements and as it's impossible to make this more comprehensible (at least to me and as of now, this guide grew so much over time and there's another chapter I want to add later so it's a whole lot of info) - please use search function if you're lost. I tried to list everything and everyone when and where I first saw them but it's still random. There's also a separate section for lore and other fun stuff you can try and explore outside of achievements at the end of this guide.
And I want to thank RainingLamppost for beaing with me asking loads of questions and trying to collect all of this, for making this game, obviously, and for being so so nice all the while. And Ultra for confirming and clarifying some extra things I wasn't sure about. This wouldn't be possible without you, quite literally. Thanks :3

All done, finally. Will replay this sometime later but now I'm definitely done, the game is amazing though.
Stay safe out there and hope Friend Shape is not bothering you too much :3
Achievements outside of Digressions

Realitas Ficta, Creatio Verus - you got this one if you launched the game but just in case Encounter a Sign at the very beginning. The pic above, basically.

Mal de Coucou - on your way to Backwoods aka your first obligatory Digression Encounter Hifive to get some info. Talking to them afterwards is a good idea too.

You Dropped This - you can meet Edmonton in the Mall (East) after the intro, pretty much unmissable.

Mauerbauertraurigkeit Semaphorism - ask Micheart Jinsil about themselves, the very last option in the Context. Apparently there's a 50/50 chance to get this achievement when you do so (I got it on the first try but was corrected by the developer, thanks!)

Forelsket Heartworm - meet Someone. Again, unmissable.

Catoptric Tristesse Cafuné - Talk to Mr Dac (Mall) after completing two Digressions.

Mimeomia Lachesism - meet Lurker at the door on the Mall second floor, they'll ask you for 4 items. This is your chance to revisit all 3 previous Digressions and get access to Hallways. 2 items are in Backwoods, you'll need to Encounter Zygoma for one (Shawl), stumble upon the other (Hair). One is in Axenic, you saw it before but now you can take it (Shoes) and the last one is in SCP, you'll find it on the wall after getting Jouska (Dress). This is also nessesary for Closure later on.

Ruckkehrunruhe - after Hallways check the very bottom of TradeMark's item selection, there's a note that costs 80 Credit which leads us to...

Filing Cabinet - just checked - everything works, don't forget to get the one from the very end of the game and you're good to go. You need to Encounter most shapes before you get their files, the outcome doesn't really matter.
Found files count: 20, that's all of them.
Here's a full list:
-Welcome 2 Levalley (2), unmissable
-The Works Of Dr Ponder Winchmore (7)

  • On Blood Shadows (in Backwoods, crooked tree path)
  • The Collective (Axenic, obv need to meet Shape in question. Now this is the trickiest one as you can't really see it in the corner of a square swimming pool room you walk around but you'll get stuck there if you stumble upon it. There'll be a screenshot below this list for good measure, Glad is in lower right corner)
  • Malafanto (in Backwoods, Jellyfish maze)
  • Malsona (Axenic before you enter the Digression, need to have Malafanto file first)
  • An Account of a Slasher and a Gatherer (Hallways, after you see text events for both Slasher and Gatherer)
  • Huh?Mans A Study (bought from TradeMark)
  • Huh?Mans, An Update (in Backwoods, Hut)
-Misc (6)
  • On The Nature of Digressions (SCP, place with an empty pool-like space in the middle and\or color puzzle)
  • Upward Machinations Poster (Mall, on the wall across from Mr Dac)
  • Micheart Jinsil File (SCP, walkway with nooses in the dead end)
  • Scribbled Note (the one that's from the Lurker after Basement)
  • Fun Factz (bought from TradeMark after Axenic is completed)
  • Another Scrawled Note (file from the Endgame)
-Newspaper Scraps (5)
  • Advertisement Page (Mall notice board)
  • Torn Newspaper (the very beginning of the game, hard to miss)
  • "The Rise of Anarkio" Article (Mall notice board)
  • Scrawled Newspaper (Mall notice board after you finish Basement)
  • Sentences (Mall notice board after you finish Basement)

Era's End Entities Encounter Expert - now if you prefer to play prepared go watch it, it's not yet finished but there's a lot of explanations and lore, if not - you can always watch it later but this list will be confusing. Now some of these Encounters are text events, those are not underlined. For any extra specifics there's a list of Shapes and lore- or rather Shape- related text events go to the last section of this guide. Also if you're unsure about Encountering a certain Entity you can check with Micheart Jinsil, the list of Entities you can ask them about is not full but still extensive.
The list is as follows and again thanks to RainingLamppost for this:
Abandoned Mascot (SCP, Hallways)
Abrogabration (Hallways, Axenic)
Blood Shadow (Hallways, Backwoods)
Conceptual Parasite (Axenic, SCP, Basement)
Disquieting Figure (Backwoods, SCP, low Sanity)
Egosystem (Axenic, text event that leads to an Encounter if you open the door Normal Person went in)
Gatherer (text event in Backwoods, the one with a giant something that ignores Glad)
Huh?Man (Backwoods on the left of flesh Huh?House, SCP occasionally)
Pallidrome (Basement)
Psych (Axenic, SCP, Hallways)
Reflection (Axenic, unavoidable)
Rejectamenta (Backwoods, bag you can see on some levels)
Scrapes (the one you save your game with)
Slasher (text event in Hallways hence why not underlined)
The Becon (Basement)
The Collective (Axenic, pretty much unavoidable)
The Friend Shape (everywhere except Basement, need to open the door first, you can guess the one in question)

Now if you did this all the last fix should give you an achievement in about a minute after you launch your save. It's working now.
Backwoods

Now some things that are needed for a collection achievements:
-keep an eye out for the hut to get Notes as part of Filing Cabinet.
Also at some point there's one half-visible note on the ground while there's a sprite that looks like Micheart Jinsil hiding behind a tree, a short trail with crooked trees and nothing much else.
And yet another one in a maze with jellyfish, it's quite visible on the ground. All three will be in your Archive under "The Works of Dr Ponder Winchmore".
-Blood Shadow can be found here, if you're collecting the notes there's a whole backstory to them too. Particulars are listed in the Hallways section.

Occhiolism - when you see something resembling a head\glistering choose to interact with it. There are different ways this interaction can go, as most in this game do but to get this use every option at least twice and when the first four stop raising the meter - Withdraw. Could also be just some mushrooms in the ground you can interact with.

L’esprit de L’escalier - you'll see a bag occasionally (looks kinda like a table on the map to me) - open it. Insist on opening it. If you don't succeed it'll show up again so no worries, but this will give you something you'll need later for two achievements so I advise you to open it sooner rather than later but note that you won't Encounter this Entity after you open them. This is also part of Era's End Entities Encounter Expert.

Greng-jai - you'll see a house of flesh, simply go left (going right will avoid it entirely) and meet Huh?Man, only use Context to assure Huh?Man all is in order and don't go to Huh?House. This is also part of Era's End Entities Encounter Expert.

Gigil - help Silent Alarm (Context again), use Niap if you can spare some, they'll give more back after.

Adronitis Amica - Encounter Zygoma and be nice to them, give Meateor (Context) and just be friendly, they don't mean harm and have something you need. Initially this Encounter is part of yet another achievement but you can Encounter Zygoma without it too (thanks, Ultra!), as in before you get the item from them the Encounters are possible, after that - no.

Boketto - Encountering Disquieting Figure needs low Sanity, will be possible either way too but you need to have low Sanity first. Just use the first four options and you're done. This is also part of Era's End Entities Encounter Expert.

Mangata - while Encountering The Timeless Chorus insist on Touching it multiple times. That's it.

A Sun Submerged - you'll get this after a successful interaction with The Timeless Chorus.
Axenic

Now some things to keep an eye out for:
-you'll see shoes on the floor on a certain level, you'll need them later;
-can Encounter Abrogabration here too.

Rubatosis - there's a chance to find a boarded up door with 'The Friend Room' plaque above it. Remove all three boards, see what happens. If it didn't trigger first try you'll need to interact with that door again, it'll come up if opened, no worries. Please note that opening that door is irreversible and will unleash The Friend Shape upon the world, they damage you greatly and can be a huge pain to Encounter. You can do it later to save a lot of time but this action is also connected to other achievements so ultimately it's all up to RNG and you willingness to Encounter a certain Shape over and over and over again. On the other hand you will see more text events this way as now getting to The Friend Room doesn't count as Digression progress. This is also part of Era's End Entities Encounter Expert.

Iktsuarpok Dokokashira - when you Encounter The Threshold of, which is basically a door out of nowhere standing idly and blocking your path, don't open said door. Just don't. Nope.

A Jacket to be Cast Off - when you Encounter Corpse use Niap, Clothes Rag, Warmth of Another and Speak. Speak should be last, everything else doesn't matter.

Xeno Keyframe - in Encounter with Psych you need to fill the blue meter. Observing and Touching makes them come closer, Gesture and Speak makes them go away unless they're at max distance already. The trick is to make them come closer twice, say use Observe and Touch, then Gesture and Speak, don't rush it, unless you fill orange by accident you can experiment till you up blue meter twice with Psych at max distance. Now use Cloth Rag on them. You're welcome. This is also part of Era's End Entities Encounter Expert.

Vemodalen - have to fill the orange meter while Conceptual Parasite is non-existing nearby. Use Withdraw first, then anything but Touch and items. This is also part of Era's End Entities Encounter Expert.

Monachopsis Chrysalism - there will be a text event about a Normal Person, open the door and Encounter Egosystem.
1. Touch
2. Speak (but be careful not to get Rare Show of Force here, can possibly be interchanged with Gesture)
3. Withdraw
4. Gesture (can get it right here bu just to be safe two more options that worked for me)
5. Observe
6. Observe

Thanks goes to Octuku for figuring it out and Celtic7Guardian for double-checking. This is also part of Era's End Entities Encounter Expert.

Rare Show of Force - while already in Encounter with Egosystem (see above) spam Speak till you see 'Egosystem implores you to join the human race, asking why you can't join right now' - use Speak once more and you should be good.

Fitzcarraldo - when you encounter Illinveiglecium don't use Niap on it, just Observe, don't interact with The Collective. This is also part of Era's End Entities Encounter Expert.

Anecdoche Fitzcarraldo - while in Encounter with The Collective use first options (only interact with them, not Illinveiglecium) till you raise orange twice. Withdraw. That should do it. This is also part of Era's End Entities Encounter Expert.

Paro Backpfeifengesicht - while in Encounter with Reflection use Pain and Stupid (Context) as much as you can. This is also part of Era's End Entities Encounter Expert.

A Weakness Embraced - you get this after a successful interaction with Reflection.
SCP (Soft Contained Play)

Now some things to keep an eye out for:
-there's a chance to find a Huh?Man here too but Backwoods is more likely;
-Disquieting Figure shows here too;
-Psych is in hiding here;
-in one of the areas with ropes hanging like nooses (and a PAIN poster) keep an eye out for a note for Filing Cabinet.

Ambedo - Encounter Belarone and play along (Context) to dance with them. This is also part of Era's End Entities Encounter Expert.

Putra Dancante - second Encounter with Belarone, I'm not sure there's any other outcome possible so let's say you'll get it as long as you Encounter them, I had no luck with my oldest save but loading on old pre-Basement one worked wonders in case you're having a similar issue. Might as well add that the only thing that seemed to work was a trusty Cloth Rag but it didn't change much. This is also part of Era's End Entities Encounter Expert.

Enouement - interact with Protoplego and fill blue meter before they disappear, it's way easier after the update but if you want a 100% chance the list of interactions that raise blue meter is as follows (I noted only last words from phrases so it's not too long:
0. Niap (the only thing to give blue right off the bat)
1. [Music] - Speak
2. [Applause] - Gesture
3. [Music] followed by [Applause] - Speak
4. hello - Speak
5. bye - Gesture
6. mistakes - Warmth of Another (you're probably done by now but I didn't make this for nothing after all)
7. foreign life - Touch
Rinse and repeat. This is also part of Era's End Entities Encounter Expert.

Sum Hic, Liberos - Encounter Abandoned Mascot, you apparently need to fill the orange path, have to figure that one out all over again. Blue path and the extra path below won't work, stay tuned. This is also part of Era's End Entities Encounter Expert.

Non Possum Hic Esse, they're here or in Hallways. Please someone check this but so far this is the only way I have. Now you need to spam Touch\Withdraw till tey reach out and you get a "Pull on Hand" option in Context, do it. Then Observe\Gesture till they tear their head off. Be careful not to fill the Blue meter though, as that will give you a different outcome. This is also part of Era's End Entities Encounter Expert.

Jouska - only available on your second visit, necessary to unlock Hallways. I'm unsure if you need a certain item to do it but just to be sure I mentioned it in Backwoods as it's there. In case you're unsure which item I'm referring to, that's the Broken Mace Attorney Figure you get from opening Rejectamenta. You'll also get Plastic Hat afterward and both items will be needed for another achievo quite soon.

Altschmerz Rigor Samsa - support Edmonton and accept them. They're not very privy to Being Touched so use Speak and Gesture unless they're prompting you to do otherwise. Can't be done with the next one.

L’appel du vide - just spam Withdraw during Edmonton Encounter.

A Glee Rekindled - you get this after a successful interaction with Edmonton.
Hallways

Can also Encounter here: Abandoned Mascot, The Threshold Of, Corpse (thanks again, Ultra).

Kuebiko - give Blood Shadow Silent Blood or use Pain on yourself, don't antagonize them and they'll eat and go on their way. This is also part of Era's End Entities Encounter Expert.

You're No Fun - please note that this is only available after you got the file On Blood Shadow, see Filing Cabinet achievement for details, at this point you'll be back in Blackwoods probably, but as to not make this more confusing than it already is please bear with me here. Anyway: give Blood Shadow Silent Blood (Context) to open another option - giving them Faex. Insist on showing it their way and as pretty much anyone else in this world they'll connect the dots and think you're a jerk. If you don't know what that means read the file or watch Era's End Entities. This is coincidentally a part of Era's End Entities Encounter Expert.

Sonder Kairosclerosis - use the first option in Context repeatedly with Abrogabration. This is also part of Era's End Entities Encounter Expert.

Kindofoods Clew Member In-Training - Abrogabration Encounter gone wrong. As this is from a not-yet-aired episode of Era's End (and not the existing one as I thought), Raining spoiled to me that getting this needs using Pain or Niap on them (Abrogabration, not our irreplaceable game developer). Pain works but is rather cruel, interestingly enough Niap is even more cruel when you think about it, unsure if Niap will give the achievement but it should, Pain does for sure. This is also part of Era's End Entities Encounter Expert.

Closure - okay, this is where it gets complicated. As in three-part, multiple steps kind of thing.
So firstly you'll need Rejectamenta (L’esprit de L’escalier achievo from Backwoods) to get the Broken Mace Attorney Figure, then get Jouska while using said item on Dr Hydra to get Plastic Hat and finally in the room after the last Hallways Encounter you need to pick up the Birthday Photo which apparently refuses to be picked up by controller, mouse clicking works wonders once more!
Next you have to revisit the entirety of Hallways to Encounter Pursuant once more and use all three items from Context, order doesn't seem to matter. And now you're done.

Avenoir Saudade - unmissable (unless you die, well you can't but you get the idea).

A Mystery Considered - complete Hallways.
Basement

Now here's a lot of text. Basement is very long, gets really frustrating and has a bunch of stuff. This is why I recommend saving every chance you get. Please remember: whatever it looks like and seems like, it's working, there is a solution to find and it will eventually end (I checked four times now!). If you're stuck somewhere there's a full video walkthrough below too. Please note that it's nearly impossible to get everything in one run so you'll need to revisit.
Also you'll be asked something at the start of each part and if you want answers to that go to Opia Orenda in Part 2 of this section. Also SPOILER TERRITORY!

Part 1

Daguerreologue Nodus Tollens - don't use Pain or Withdraw with Empty Space.

Kairosclerosis - Encounter x v zA. This one is unmissable so far.

Memrekurso  - Encounter Æ_. Missable due to RNG only.

Dialecstatic - Encounter $CF13. Missable due to RNG only.

Etterath - Encounter M p'u ウ. Missable due to RNG only.

Depaysement - answer all 26 questions correctly. Can't be done with the next achievo.
Now I probably should say that some of these questions can be answered only after you complete Basement and some can't be answered (without randomly guessing) at all. I will give you a hint though. There are 26 questions total, now look at the first few answers and that's your hint. You can either try yourself or follow the list as it goes.
1. ABRIDGE
2. The Basement
3. The Collective
4. Mr Dac
5. Elyrisitch
6. Fritzial
7. God's Appendix
8. Dr Hydra
9. Illiveiglecium
10. Jinsil
11. Kied
12. Back to Leavalley
13. Micheart Jinsil
14. Niap
15. OEN
16. Pymme running away
17. Quaffee
18. Rejectamenta
19. Soft Contained Play
20. The Threshold Of
21. Upward Machinations
22. Vincenzo
23. Aegrum Whilt
24. x v zA
25. Yazmina
26. Zygoma

The Beckon - Encounter The Beckon. Now the trick here is to answer as many questions as you can with 'The Beckon'. That option comes and goes but it's there from the very first question so pick it and keep picking it. I'm not sure it matters but when I couldn't pick it I chose wrong options on purpose. This will lead you to the most elusive Encounter in the game (IMO, obviously). This is also part of Era's End Entities Encounter Expert.

Numb Melancholy - Encounter The Pallidrome. Now this is random but there are also conditions to be met. The Pallidrome is basically The Basement version of The Friend Shape, meaning you have to open The Friend Room to Encounter them. Being Marked (it's a text event in Hallways) will help but from my experience not by much as I only found them on my third Basement run. Heads up RNG hates me so you might get lucky. Opening The Friend Room is still a must either way. This is also part of Era's End Entities Encounter Expert.

Part 2
Opia Orenda - get every word right in Encounter with Elyrisitch. This is a tricky one and also why Basement is divided into two parts here. Don't worry if you miss some but note that wrong answers drain your stats. Here's a full list of words in the order you'll need them:
  • The practice, sacrifice to face the almighty
  • Trying to make meaning of it all (could also be 'sense' here, whichever works)
  • It was not to come to pass
  • Disoriented essence
Now I'll just list the words in order you'll need them, divided by verses.

Answer, disease, solitary flier, solitary one.

Empty, bone-enclosure, lamented, (painful, bitter), journey.

Answer, simple, suffer, bear.

Adversary, malice, forever, monster.

Intending evil, deceive, anywhere, present, burst.

Abandon, heresy, dripping blood, misery.

To call, to fall, to come, to know, grief, toil, clear, to find out.

Answer.

Exulansis - ABRIDGE can be Encountered at the very last part of the Basement, unmissable.

Se Dio Volas - unmissable, won't say more.

A Memory Burned - survive the entirety of Basement. That's it.
Basement, video walkthrough
Please, please note that it's my fourth visit to the Basement and I get a bunch of events you won't see unless you followed this guide beforehand and they're still random. I purposefully missed an Encounter (but pointed it out) and in general this is for those who got lost in the Digression itself.
For reasons I have no idea about the sound is missing everywhere and that sucks but as this was meant to be an interactive map of sorts so maybe that's for the better. So yes, Basement not scary, everything solvable, enjoy that one Encounter you didn't ask for and probably haven't seen like ever.
And just in case I asked for permission to do this, obviously. Thanks, RainingLamppost :3

Endgame

Liberosis - you'll Encounter Malkin either way, Withdraw.

Borem - you'll Encounter Malkin either way but this time just stare at them, use anything but Withdraw.

Alexithymia - unmissable.

Ellipsism - again, unmissable.

Realitas Ficta Creatio Verus - that's it, folks, all done. Unmissable.
But what does this mean?..
No lore spoilers here but it's a somewhat deep-lore, complicated thing that won't mean anything unless you're familiar with the story, characters and lore. I want to talk about something lore-related, hidden in plain view. Have to admit I got a ginormous clue from Ultra as to where to look but I did dust my rusty Latin and noticed the pattern itself (Japanese helped lots too, being multilingual just made me go 'okay, this is definitely not random'). So without further ado, the achievements and all tracks from the soundtrack have meaning. In most cases it can be found in The Dictionary of Obscure Sorrows by John Koenig (for clarity I used an abbreviation TDoOS and didn't quote full entries, just some excerpts, you can always look into it yourself). Please note this is purely speculative on my part and I probably got something wrong or incorrect but it was fun to try and decipher this.
The current list as you see it is full soundtrack as there's more to see than in achievements alone.

1. Klexos
Your life is written in indelible ink. There’s no going back to erase the past, tweak your mistakes, or fill in missed opportunities. When the moment’s over, your fate is sealed. But if you look closer, the ink never really dries on any of your experiences. They can change their meaning the longer you look at them. (TDoOS)

2. Realitas Ficta, Creatio Verus
Fictional Reality, True Creation
Or to be more poetic and possibly a tad incorrect, my Latin is rusty indeed: Reality is fictional, creation is true\real (Latin)

3. Ruckkehrunruhe (TradeMark)
The feeling of returning from an immersive trip only to notice it fading rapidly from your awareness, as if your brain had automatically assumed it was all just a dream and already went to work scrubbing it from your memory. (TDoOS)

4. Vellichor (CityScape)
The strange wistfulness of used bookstores, which are somehow infused with the passage of time—filled with thousands of old books you’ll never have time to read, each of which is itself locked in its own era, bound and dated and papered over like an old room the author abandoned years ago, a hidden annex littered with thoughts left just as they were on the day they were captured. (TDoOS)

5. Mal de Coucou (Hifive)
Condition in which you have an active social life but very few close friends—people who you can trust, who you can be yourself with, who can help flush out the weird psychological toxins that tend to accumulate over time—which can eventually progress into a state of acute social malnutrition, where even if you devour an entire buffet of chitchat, you’ll still feel pangs of hunger. (TDoOS)

6. Waldeinsamkeit (The Backwoods Digression)
That particular feeling of being alone (and lonely) in the woods. (German)

7. Boketto (Disquieting Figure)
To stare into space absentmindedly, to do and think of nothing. (Japanese)

8. Gigil (Silent Alarm)
Overwhelming emotion, usually caused by anger, frustration or cuteness.
Or, alternately: that uncontrollable feeling to squeeze something very, very, cute. I'm not familiar with this language so I listed both but guessing the latter. (Tagalog)

9. L’esprit de L’escalier (Rejectamenta)
The predicament of thinking of the perfect reply too late. (French)

10. Adronitis Peregrinus (Zygoma)
As you may see below it's a three-part puzzle.
Adronitis - frustration with how long it takes to get to know someone—spending the first few weeks chatting in their psychological entryway, with each subsequent conversation like entering a different anteroom, each a little closer to the center of the house—wishing instead that you could start there and work your way out, exchanging your deepest secrets first, before easing into casualness, until you’ve built up enough mystery over the years to ask them where they’re from and what they do for a living. (TDoOS)
Peregrinus - stranger, foreigner, traveler etc. (Latin)

11. Adronitis Figura (Zygoma)
Figura - if I were to guess here the correct meaning would be 'Shape'. Could be translated as 'Figure\Form' too. (Latin)

12. Adronitis Amica (Zygoma)
Amica - Friend. (Latin)

13. Occhiolism (Excogitiomycota)
The awareness of how fundamentally limited your senses are—noticing how little of your field of vision is ever in focus, how few colors you’re able to see, how few sounds you’re able to hear, and how intrusively your brain fills in the blanks with its own cartoonish extrapolations. (TDoOS)

14. Mangata (The Timeless Chorus)
The reflection of the moon on water. (Swedish)

15. Kairosclerosis Nighthawk (NightScape)
Kairosclerosis - the moment you look around and realize that you’re currently happy—consciously trying to savor the feeling. (TDoOS)
Nighthawk is, obviously enough, a nocturnal bird. The only proper reference I can think of here is a short story The Nighthawk Star by Miyazawa Kenji.

16. Forelsket Heartworm
Forelsket - the euphoria and elation experienced when falling in love, often characterized by a sense of blissful enchantment. (Danish\Norwegian)
Heartworm, unless referring to a rather nasty parasite (which I doubt it does, at least not literally) doesn't need translation.

17. Catoptric Tristesse Cafune (Mr Dac)
Catoptric Tristesse - the sadness that you’ll never really know what other people think of you, whether good, bad, or if at all. (TDoOS)
Cafune - the act of running your fingers through someone's hair in a tender way. (Brazilian Portuguese)

18. Onism (The Axenic Digression)
The awareness of how little of the world you’ll experience. (TDoOS)

19. Sonder Kairosclerosis (Abrogabration)
Sonder - the awareness that everyone has a story. (TDoOS)
Kairosclerosis - the moment you look around and realize that you’re currently happy—consciously trying to savor the feeling. (TDoOS)

20. Vemodalen (Conceptual Parasite)
The fear that originality is no longer possible. (TDoOS)

21. Monachopsis Chrysalism (EgoSystem)
Monachopsis - the subtle but persistent feeling of being out of place. (TDoOS)
Chrysalism - the amniotic tranquility of being indoors during a thunderstorm, listening to waves of rain pattering against the roof like an argument upstairs, whose muffled words are unintelligible but whose crackling release of built-up tension you understand perfectly. (TDoOS)

22. Fitzcarraldo (Illinveiglecium)

Rigor Samsa - a kind of psychological exoskeleton that can protect you from pain and contain your anxieties, but always ends up cracking under pressure or hollowed out by time. (TDoOS)

*wow, now I know the character limit for this*
But what does this mean?.. (2)
31. Greng-jai (Huh_Man)
Unwillingness to take advantage of someone’s good nature. (Thai) This one is a cultural concept kind of thing so it's very hard to translate and I'm barely familiar with Thai language but this is the gist of it.

32. Xeno Keyframe (Psych)
Xeno - the smallest measurable unit of human connection, typically exchanged between passing strangers. (TDoOS)
Coincidentally it's also Greek for 'stranger', like in 'xenophobia' and also (thanks to one pretty huge franchise and the internet being the internet) 'alien' as in 'xenomorph'. A lot of languages don't distinguish those two much.

48. Iktsuarpok Dokokashira (The Threshold of)
Iktsuarpok - the feeling of anticipation while waiting for someone to arrive, often leading to intermittently going outside to check for them. (Inuit)
Dokokashira - now this has so many meanings I just went and asked Raining directly, so generally the best general translation I came up with is "Where could it be?", "it" in this case being pretty much anything or anyone for that matter. I kinda like the possibilities it opens for interpretation if you're familiar with that particular Shape. But in case you want to know what it was meant to mean: "Where could this door lead to?". Shapes and other Encounters

Since listing achievements is confusing this is an even more work-in-progress section but I want to make a list that should in theory help find whatever it is you Encounter. And also note where I Encountered them and occasionally what I did if it's significant. No articles so it's easier to navigate and no Entities at the end of Digressions or those you can't avoid and\or I have nothing to say about.

Abandoned Mascot - blue path is filled by using Clothes Rag each time the text says they've been sick in the suit and Gesture when they start clawing on the suit. Coincidentally tearing their arm off (done via Context)fills orange once, nothing else did so far. (SCP, Hallways)

Abrogabration - they're pretty chill. Hitting them with Pain ends the Encounter right away if you wanna be cruel about it. (Axenic, Hallways)

Blood Shadow - give Silent Blood or use Pain on yourself to feed, they're quite amusing. The one in Backwoods has a backstory too so you might want to read that and maybe use something different, like Warmth of Another on them, do so twice and you can show them their file from new Context action. Was that the right thing to do?.. Also works on Blood Shadow in Hallways.. You might also want to try and use Cloth Rag on them and then yourself which will have two different outcomes depending on your wellbeing. Use Faex to shoo them away but they won't like it and it only works once. (Hallways, Backwoods)

Collective (The) - most agree they're really nasty. If you want to, you can use Niap on Illinveiglecium and be on your way. There is a better outcome to this, however, I listed that for Illinveiglecium entry below. (Axenic)

Conceptual Parasite - you can just wipe them with a Cloth Rag. Really. (Axenic, SCP, Basement)

Corpse - don't let them know they're no longer alive. A corpse. Dead. Jinsil will say it's your corpse but we know how Jinsil can be, don't we? (Axenic, Hallways)

Disquieting Figure - needs low sanity to find but can show up regardless occasionally, that screaming from the hut in Backwoods, that's them. Blue (actions) or orange (items) paths both lead to the same result. (Backwoods, SCP)

Egosystem - text prompt to open the door where the Normal Person went. Don't open to avoid. Give Niap to melt them quickly. (Axenic)

Empty Space - unavoidable but if you're revisiting and in a rush - hit them with Pain, won't be pretty as it drains your stats but will be fast. (Basement)

Excogitiomycota - the clever fungi, can be avoided first upon encountering something like a head, later it's just three mushrooms on the ground, don't touch if you don't wanna. (Backwoods)

Friend Room - not an Entity, a boarded door. Don't open unless you really want that achievement or love yourself a challenge. (all Digressions, even Basement)

Friend Shape - run. Just run. I know you can't so just note that no item you can use will remove that debaf, you just have to wait 3 turns, no other way. Context is useless, items give some variety to text but that's it. (EVERYWHERE except Basement. But if you did open that door there's something else waiting for you in the Basement too, just not Friend Shape)

Huh?Man - can be avoided in Backwoods by going right. I tried agreeing to go with them but it doesn't work, no making Huh?House a Huh?Home for me. Can be Encountered by stepping on thin orange cord in Backwoods as well. If you don't wanna deal with them give Warmth of Another and be on your way. (Backwoods, SCP)

Illinveiglecium - can't be helped much, used to lure you to The Collective. If you really want to, you can distract The Collective with Niap long enough to snatch Illinveiglecium away from them. (Axenic)

Micheart Jinsil - didn't expect them, did you? I'll repeat that talking to Jinsil will reveal most Shapes you've seen so far and also outcomes of said interactions (be they Encounters or text events). Now there's an old joke that says the chance to see a dinosaur is always 50\50 - you do or you don't. And that's Jinsil in a nutshell. Take whatever they say with a grain of salt, preferably with a teaspoon of it. But the info can be helpful nonetheless. (everywhere but Basement)

Protoplego - nothing much left. (SCP)

Psych - just ignore them. Really. Use Niap to get rid of them in one turn or simply Withdraw. (Axenic, SCP, Hallways)

Rejectamenta - a bag. Open this one. Do it. (Backwoods)

Silent Alarm - must be protected, gives 3 Niap even if you're too cheap to use one on them. Apparently you can refuse to help them but I'm not doing that, you're on your own there and I may or may not tell on you to every Blood Shadow I befriended. Which is all of them. Or simply remind a certain someone about a cool way they dealt with pouchers back in the day and pay to watch. (Backwoods)

Threshold of (The) - another door. Anyone else getting traumatized by doors in games? Don't open. I don't know what's behing that door but if you want to open it I got you covered. No responsibility for what'll happen though! Now the recipe for disaster is as follows: Observe, spam Context till you know it's locked. Withdraw, it opens. Now spam Context again till the blue meter is full. Enjoy your doom, muah :3 Interestingly when you open The Threshold of there's no knocking on the door in Hallways as soon as you enter the Digression. (Axenic, Hallways)

Zygoma - can be befriended but won't play fetch. You can only Encounter them on a quest from Lurker or before you get said quest, once the quest is done they'll be gone. (Backwoods)

Text events

Now while most either lead to Encounters or result in using an item or trying your luck there are some that are... different.
Also if you're after extra lore a list of posters you might wanna check out:
-two on both sides of Mall main entrance;
-one right there on the ground;
-Whales and Orphans (Axenic), has a whale on it, it's a direct Era's End cross-reference;
-a summary for a comic in SCP digression (looks a lot like Cowcium poster, top of the long slide);
-Cowcium poster in SCP (red with a skull on it);
-Bunker Dwellers poster in SCP (looks like a black square in the room with something like an empty pool on the floor, need low stats to see);
-Children Ascension poster (SCP, looks like a red circle with black markings, in a maze with sandbag-like thingies, whatever they are).
Also you might want to read item descriptions from your inventory if you didn't already.

Backwoods
Silent Stalker lore (mentioned twice)

Axenic
The Oscillating Nonsense lore.

SCP
Zach (Anarkio) lore. Finally found some more!
Merchy occasionally turns up, they will heal one stat for free first time. (Shows up in Context for Jinsil, interestingly enough)

Hallways
While traversing Hallways in the text bit you may find a door you can't pass as it spews black goo at irregular intervals. The game prompts you to use a rag or try and brace it but the latter comes with '(???)' and if you choose it you get "Marked" which is written in red. No clue what this will lead to but on my next visit the game said it means something can find me easier. Hope it's not more Friend Shape. But it is!

The Cheeky Chappy (whatever they are) rarely appears, it says you need low health but I had 47/55 and nonetheless got this

While there's multiple mentions of Slashers in the game, there's a text event where you actually see one.

Basement
There's a text event at the very beginning where it's still 'Backwoods' describing Bone Traitor.

No problem, I'm well aware how hard it can be to Encounter Shapes you need, glad it worked and thank you for getting back with this, it helps a lot, I'll update the guide.

As for Numb Melancholy it's random, I had that particular Encounter right after the questionnaire (you can check it in the video too: https://youtu.be/ACAypMvp-vs?si=WW_XKb4Bim41ISs_&t=2048 if the link will work). Also met them in the later part of the Basement once, mostly it's as with certain other Shape The Friend Shape or also Egosystem but that one gives you a choice - there's a chance to Encounter them at any given time during the text event\transition. As I wrote in the guide it took me 3 full runs to get this one and I had increased my chances beforehand (the text event itself is explained in detail in the Text events section right above this message if that helps).

Sorry for how late this response is! It took several tries to get the encounter to show up again and I finally got it to appear a few minutes ago. I went Touch, Speak, Withdraw, Gesture, and then automatically escaped and got the achievement. I did not need to observe twice. So yeah, it did work this time, thanks for the help!

One other question, this one being about Numb Melancholy. At what section of the Basement is this Shape likely to appear? Unlike most Distortions, it feels like the Basement has a lot of 'set' areas rather than randomization, so I'm not sure where that Shape could show up in place of the 'not random' parts.

Please do, from what Octuku mentioned I think it should be fine to switch 2 and 4 moves as to not get Rare Show of Force again but just to be safe can you maybe record the whole Encounter? So that if it idoesn't work again I could double-check?
Celtic7Guardian Jun 10 @ 6:27pm
Good to know that's what I'm looking for, I'll try that exact method. The first time I ever attempted it I did speak and accidentally triggered Rare Show of Force, haha, but I didn't mention that earlier because I knew why that didn't work. Thanks for the help and I'll say how it went after I manage to find it again. :fhappy:
MadHatterBelial  [author] Jun 10 @ 2:01am
Weird, you should be able to end that Encounter on turn 6 by Observing with the text saying Egosystem mimicked your movement and you escaped by going around them. I did pretty much what I listed:
1. Touch
2. Speak (but be careful not to get Rare Show of Force here)
3. Withdraw
4. Gesture
5. Observe
6. Observe
Celtic7Guardian Jun 9 @ 8:22pm
I mean that I withdrew on turn nine because the method listed under Monachopsis didn't accomplish anything. Turn six went by with me observing, then turn seven and eight were seemingly pointless. I had to use the withdraw command on nine to escape because otherwise it'd have murdered me. Was something special supposed to happen on turn six when I used observe?
MadHatterBelial  [author] Jun 9 @ 4:55pm
By withdraw do you mean the text saying you got around them and escaped while you didn't use Withdraw action itself as it has to be Observe on turn 6?
These are the only 4 outcomes (besides the one that ends in failure\death) I found: Rare Show of Force one, the one listed under Monachopsis Chrysalism and Withdrawing at the very end (9 turn, I think) where you dodge under their arm, also Niap that just melts them and does nothing.
Celtic7Guardian Jun 9 @ 4:23pm
I'm doing some clean-up for achievements and I can't get Monachopsis Chrysalism to work at all. The steps listed don't seem to do it. I have ended the encounter each time after the sixth turn when it's close and allows you to withdraw to safety. Is there a different method I should be using?
MadHatterBelial  [author] Apr 17 @ 11:17pm
Done, thank you.
