Injuries
As listed in the databank under ALIMENTS to help you find what you are missing with some information on how I got them. Most are from fights and storms. If you have any other ways or can update info, please leave it in comments for others. Thanks.

1. Broken Claw (happens in fights. Recommend Velociraptors vs dilophosaurs in sandbox mode)
2. Broken Wing (anything in the aviary can get this from fighting)
3. Concussion (injury from fighting)
4. Damaged Tooth (Fight injury)
5. Frostbite (snow/blizzard storms or Pennsylvania campaign map when recovering the Diplodocus)
6. Hypothermia (Snow/blizzard storms)
7. In-grown Claw (Dino must get Broken Claw first. DO NOT TREAT and it may turn into Ingrown Claw. Took me about 3 hours. Tested on a friends and got within 10 minutes)
8. Internal Bleeding (Fight injury/ injury from hurricane storms)
9. Laceration (From fighting or attacking fences. Collect the Brachiosaurus in Story mission Pennsylvania)
10. Major Fracture (From fights or the Allosaurus in story mission 2 Washington State)
11. Minor Fracture (from fights)
12. Organ Trauma (from fights)
13. Second Degree Burn (Unsure but think it was obtained in campaign. Maybe storm or from a rescued dino?)
14. Sprain (from fights)
15. Surface Wounds (from fights)
16. Swallowed Object (Let a large carnivore eat guests)
17. Swallowed Phone (Let a large carnivore eat guests. Story mission California Albertosaurus )
18. Third Degree Burn (Unsure but think it was obtained in campaign. Maybe storm or from a rescued dino?)
Diseases
Since Germany (Jurassic Difficulty) in challenge mode has super high disease chance, I got a few there when trying to beat it. Please leave comments if I'm wrong or you know other ways to obtain these. Trying to do this from memory is hard. Thanks to GoatPlayBanjo for Rabies info! ♦§ Ichthyovenator §♦ for their input on Hyperoxalemia!

1. Algae Poisoning (Think this is from a lack of water but high needs for everything else.)
2. Avian Influenza (Lots of dinos in a small area I believe)
3. Avian Pox (Random possibly?)
4. Bracken Poisoning (From lack of forest need. I got this from Apatosaurus being completely happy bar forest I believe)
5. Chronic Stress (High levels of discomfort but seems random)
6. Coccidioidomycosis (In the campaign I think. Description says exposure to a fungus found in sand and soil)
7. Common Cold (Fairly easy if you mix dinos together. I had Velociraptors fighting Dilophosaurus for injuries list and this appeared a lot. In story mission 3 Pennsylvania I collected the Torosaurus but forgot to fill their pen with anything but water and they got this before I could remedy this.)
8. Cryptosporidiosis (Random and somewhat common. Story mission 3 Pennsylvania Ankylosaurus arrive with this)
9. E. Coli (Have a meat feeder used by several dinos. Procompsognathus (Compys) trigger this a lot. Shark feeders can also get this)
10. Foot and Mouth (Lack of open space need. Use Mosasaurus as it can't break out of the lagoon)
11. Hookworms (Velcoiraptors are susceptible to this if feed by meat feeders)
12. Hyperoxalemia (♦§ Ichthyovenator §♦ stated they got this on Germany Challenge map with an Apatosaurus lacking forest and tall leaf.)
13. Infected Wound (Don't treat any of the wound types *surface, lacerations, broken claw etc* injury and it will go infected)
14. Ivy Poisoning (All herbivores needs meet besides food I think.)
15. Rabies (Story mission California when Dilophosaurus are collected.)
16. Salmonella (Meat feeders used by multiple dinos. E.Coli and salmonella are similar set up. Think it's random which you get)
17. Silicosis (From sand storms)
18. Tuberculosis (Story mission 3 Pennsylvania Deinonychus arrive with this)

I know it's just a game mechanic, but it's crazy that the Dinosaurs can get Rabies. Then again, considering they're made in a lab there's bound to be some genetic weakening that would allow them to get infected with Rabies in the first place.
