A. There is no fee for using the Source Engine for your free mod, but there may be licensing fees for included tools if you charge money for your game.

A deposit fee applies to any small time developer or indie developer who wish to sell their games or software through Steam, so yes unless you are a big time game developer or publisher in the video game industry then you will have to go through Steam Direct.
Every title on Steam regardless of who you are pays the $100 fee.
That was not the case with giant publishers, who didn't have to go through Steam Greenlight or Steam Direct previously before Steam Greenlight was even a thing. Though I'm sure how it actually works these day because established publisher who already have a business relation with Steam prior did not have to go through a greenlight process unlike indie devs/pubs.

Steam Direct only applies to new or smaller game developers or companies who want to place their titles on Steam. Big players like Capcom, Activision, Square Enix, etc. etc. have a fixed agreement with Steam on how many game titles they plan to put up on Steam. They also usually a slightly larger cut per copy sold though it is not really that substantial unless they reach a certain threshold of copies sold.
