Has anyone found a way to disable it, it looks terrible imo

When i get in the heat of battle sometimes I get a grainy faded screen. That what your talking about? I'd like to know what's happening. I thought it was something to do with my graphics card...

it's the suppression effect, it means you are under heavy fire, makes your vision impaired and aim worse

Yes, these kinds of effects are designed in to convey your situation, damage, etc. I saw a cut scene that was grainy, but that was a memory. Its a style choice intended to make it obvious that it is a flash back in time.

Hello! Are you playing with HDR enabled in your TV/Monitor? If so, try changing the TV`s Dynamic Contrast setting to Medium/Low or Off. Let me know if this helps :)

happens to me also when under heavy fire from, say, a mounted gun, even behind cover and not taking damage

It's pretty freakin subtle and I'm pretty sure you only have it in cutscenes. But it is a useless gimmick and games should just stop using it.
