The one on very left is the outfit you unlock when you reach Combat Medic level 25. The helmet however is not exactly the same... the looking is not as cool in the game as on the picture.

The outfit is cool (i wear it when I play my medic), just the helmet... they could have make it look like the picture and it would have been better lol. I see alot of people wearing the outfit but never saw anyone with the helmet. That probably says alot! xD

I prefer to wear those sunglasses instead lol
