Is it possible you just overlooked it, like if it was the circle since that room has other floor features? Never seen it happen, has no reason to.

nope, its happened a few times on multi player, none of us could see it

I did have a friend that complained that the voodoo doll would sometimes glitch into the floor of Sunny Meadows - but when they couldn't see it, I could and I could pick it up.
friend was on console, I was on PC, though I'm not sure that was why.
