Did you get all 4 endings and all 32 bloodcodes listed here (not counting DLC ones)?

Are all 41 blood codes in your inventory at the same time (= same save file)? Are you sure you're not missing Demeter or Eos or something?
I'm using 1 (one) game file playing and playing a single character.
I would find a list of all the blood codes online and go through the ones you have one by one to make sure that you have all of them. I don't think it's possible to have two copies of a blood code, but maybe that's what has happened. You only need the 32 blood codes in the base game. I was missing most of the DLC ones when I got the achievement.
You did get the Demeter blood code from Karen's room after beating all four successors?
(Karen's room is the one after the boss room in the Cathedral). There are also two different blood codes for beating the game, Hades and Queen. If you restored Eos vestiges after beating the final boss you might have missed the Queen blood code.
