Don't duel at the start until you snowball and 2 hit everyone but their posistion 1 and 2
First your duel can used differently in lategame, either ez point farm on support OR as a disable for their annoying core (like a slark, so he can be nuked down and cant eat supports).
When duel is not available, then you have to just 'rightclick' people to death. For this its often useful to hit a good overwhelming odds (so you chase down also the spell does more damage, when it hits more targets).
As for duels, I think instead of initiating, which is dangerous, you should pick targets that are dying. Use it to get damage and not as lockdown on someone.
And kill steal is stupid. Don't care about that. The main thing is getting the duels, not who kills.
blade mail don't get that unless your vs someone with a high damage like ursa, axe, huskar
get armlet if your not vs those heroes
With that much pure AD you want as much attack speed as possible, maybe an early Maelstrom (for single target effectiveness)
Dueling at the start is fine, as long as you have allied support. It's an easy way to stop someone casting thier spells, making you/your allies have to use regen/go back to base.
TL:DR Duel at start IF you have allied damage/nukers e.g. lion, lina, ss.
and the level part, i meant the spells. what spells do you pick first on her?
