What resolution and image quality settings are you trying to play at?

I tried using the lowest resolution and lowest image quality settings and was still getting very low fps, ~20fps

Wow. I would not expect performance that bad with your setup. If you've updated your drivers, and have ensured any background applications are closed while playing and are literally setting everything on low with the lowest resolution option possible and are still getting framerates that bad, you could try opening a ticket with Ubisoft support to see if they have any suggestions and if they don't, I'd say refund the game until you upgrade your system.

Yeah, I'm going to try re-installing it on my SSD and see if that makes a big difference. If not I'll probably look into getting a support ticket. Thanks

Also try firing up MSI Afterburner and use it to monitor your GPU / CPU usage during the game and see if you notice a bottleneck.

Is your CPU really old? I know you said it's an i7 but which model?

That is because you are launching the game using your CPU's integrated graphics card. Launch the game using your GTX 1050Ti display adapter.
