BookWorm Achievement help

[Ha Ha Ha Thud!!!]

I'm trying to get the BookWorm achievement, but I'm still missing the following Vignettes: 2, 21, & 23.

I googled my fingers to the bone with nothing to show for it. All google turns up for the vignettes, or bookworm achievement just tellme that the achievement is for unlocking all the vignettes.

But how do I go about doing it?

Well, I got them all on 3rd or 4th playthrough but haven't payed attention how.
I read somewhere they are showing up randomly between some missions.

I think you need to try "all" of the starting options. I was missing a couple... tried the permadeath, and hardcore starting options and voila.
