Perhaps Valve should look into making a modded versus mode and an Unmodded Versus mode that autimatically disables any and all modifications to your game.
Then again, it might already do that for all I know.

Official servers disable all skins.
And yeah they can cause problems, easy to see infected, making bile invisible, etc etc
Versus on the other hand... No. Making special infected glow is painting a bullseye on characters that would otherwise blend into the mobs. I'm sure that it isn't possible to use it in versus though, because everyone would have to have the mod. Assuming it's like campaign's mod requirements anyways.
Not all official dedicated servers for campaign have it set to pure. There a lot of co op dedicated unpure servers out there. I have been playing on them for like 2 years now.
Ah, I don't play campaign that often other than to do custom maps (which rarely work with official) so I wouldn't really know. With co-op it really doesn't matter though, it's not hurting anyone really since everyone is on the same team. And if the team doesn't like having a cheater just kick him.
