Turn down the settings?
Although L4D2 is not a notoriously graphically intense game, do you have a GPU capable of handling it?
If yes, then disable any add-ons you have currently installed and try again.

I have this problem and im pretty sure it is a mod conflict. A workaround is when you enter the l4d2 menu quickly selct singleplayer and start a singleplayer match. Once youre in the singleplayer match you need to wait until your mods loaded (i personally exit it when i finsih the first chapter). You should then be able to play l4d2 with all your mods installed.
