It has less of an impact on us, who view this only as a fictional medium, but would have significantly more of an immediately "red flag" reaction to anyone actually in this universe.

He said in this REAL universe we are unperturbed by the implications in the fictional universe; where, if we were in fact in the Fictional Fallout universe, it would scare us as much as all the things you said... the same way it scares us in real life.
In other words while were playing the game we feel no paranoia as the player that we will be replaced by synth, although in the universe were playing in, it has been established that threat is as real as the radscorpions or deathclaws we may fight. Though, The Player does have a reaction, when we encounter a threat that would kill the player. If either threat were real in our REAL LIFE, we would consider being replaced by a synth as much a threat as every other threat, obviously.
