A good distance is about two to three or so rolls away from him. So if he does some big giant AoE you're already a fair distance away and should only have to do a few rolls to get out of there. Plus at that distance he's far enough away that you can see his attacks coming a lot easier. Just be patient and play reactively. Don't rush.
For times to heal, you'll want to wait until he's either on the last swing of a combo that you've already rolled out of the way of, or just be really careful. Don't go for two or three chugs, go for one, dodge his next attack/combo, then another chug.
Nameless King is all about waiting. Try not to rely on your shield and rather try for rolling, but if you can't get a roll off it's good to have that shield just in case.

But i beat ihm @ SL 15 with much difficulty.
#1 second phase is weakest to fire damage, but that being said i did just fine with pure physical.
#6 Yes, It may technically be cheese but it is not as easy as it sounds. Equip a crossbow or bow, maybe even both, and shoot him with arrows durirng the second phase.
He has some long charges and jump attacks to close distance very quick but once you get the dodge timing down and keep your distance you can fairly trivialize his second phase.
Dont expect this stratagy to go quickly, it took about 20 min of shooting arrows.
During the dragon fight, stay at about a medium distance away, to bait the fire breath. If he takes flight, lock on really quick and circle the opposite direction so you can dodge the lightning throw. When he comes in for the smash attack, go left and dodge at the last second. You can then run in and hit the dragon's head once or twice if you are fast.
When he is on foot, basically just stay right next to him. This will bait all his swinging attacks. Dodge his two hit combos and hit him once after his combo. If he does a downward smash, dodge and hit him once. If he flies up, dodge and hit him once. During his first phase, don't get greedy and don't block. Rely on dodging.
During his second phase, he usually charges up a giant slam attack and will 90% of the time follow it up with his lightning bolt. When he charges up, simply walk behind him to the right and then start wailing on him. Continue hitting him until you stun him. After he gets up, if he's still alive, just stay right next to him to manipulate what he does. Good luck, and seriously, try bleed if you have a lot of Luck. If you don't have a lot of luck, use Fire resins.
