I can't find underground passage or hatch anywhere.

And my perception is 9.

you need high enough perception. the left side from the church near the picture is an hatch wich will reveal itself if you have high enough perception..
but it can also bug extremly rare.

have always in one chars bag some rings and amuletes with perception or other gear. and if need use an potion on top.
