There are 2 types of islands, small and big(with high cliffs and/or large pine trees), both can vary some in size and shape, but always a lot smaller than a grid cell(there has to be some ocean between them too) :)

If we call something small and medium a lot of people will expect big to be there too whether we're talking about islands i SD or the size of wife's underwear *looks over shoulder to check wife's not looking*
I know there was a period where there were big multi-tile islands(some called these huge), but the way the game loads and unloads biomes and having a small dead zone between biomes(approx 30 floor/raft tiles), I doubt that the hugeBIG islands will return, unless they decide to redo the biome load system (again).
I never got to play with those hugeBIG multi-tile islands, but I would have loved to try it, preferably in a bigger world than we have now :)
As for huge, I know they changed how the biomes work, I am guessing they had reasons although it is a shame.
Maybe things will change when the grid world grows and the infinite world beyond the grid returns, then again maybe not heh.

Edit:Bah, ninja'd by that pesky Bushi Neko !!! ;)
