Right now , Im in basement. I spawned below a treasure room with choco milk. I just beat gemini and got kukin PENTOGRAM. and im playin as AZAZEL!!

Sacred heart, technology, guppy, holy mantle, the wafer, trinity shield, 3 pretty flies, meat cube, a bunch of other items. I beat Blue Baby and Mega Satan without taking any damage (screenshot on my profile if you don't believe me.)

Sacred Heart + Brimstone as Judas. Easiest run I've ever had.
Also Monstros' Lungs + Lost Contact made Mega Satan easy.

I've really wanted to try Brimstone + Ludo technique + Lost Contact but I can never get the items needed.

I had Soy Milk + Death´s Touch + Spoon Bender + Lump of Coal once. I was pleasantly surprised how effective it actually was.

Has to be brimstone + tammy's head + spoon bender (homing) + orbital tears

I just had a run with brimstone + Charm tears + flame tears + about 4 damage upgrades + Gimpy + Reusable eternity + revenge spider. Basically meant I could 1 hit any non-boss, 3-5 hit the stronger bosses and basically stayed at the hp cap the whole time. I could also fly since I was Azazel (I was lucky enough to pick up full brimstone)

I think the coolest combo for bigger tears than screen is this:
Polyphemus, Proptosis, Eve´s Mascara, Chocolate Milk, Sacred Heart and Godhead

Don't forget Death's Touch, Cricket's Head, Magic Mushroom, and then Use Tammy's Head.

Mom's knife followed by brimstone.
(The run was pretty much won already so I wanted to see if getting Brimstone after mom's knife worked. Yup...)
