Mostly because it's the only end boss with such an unpredictable teleportation effect combined with such attack patterns.
Delirium is a different beast, and is widely considered to be the worst boss in the game.
It's able to move around the room fast, making it harder to deal consistent damage on. It's ability to teleport around can make it hit you unreactably, especially with mom's foot.
It's tied for having the most HP of any boss in the game (though, it has different armor values depending on the form it's in.)
It can just randomly decide to spawn two to three versions of itself at once (usually because of bosses like brownie that normally spawn dangle when it dies.) And as if all of that wasn't enough, if you deal damage to it too quickly, it just massively speeds up and teleports more rapidly to screw you over.
