Since you've said the hotfix resolving the Throne II intro crash is likely coming in the next 24 hours, the top priority would probably be Y.V.'s exploding car.
Elite Shielders not teleporting would be an issue if not for their being obnoxious enough as is.
Edit: Forgot to mention, the wiki says Lucky Shot's text is bugged to say it gives less ammo than it does. Is that outdated?
It's actually pretty manageable once you can get your footing past 1-1.
If anything, the biggest issue I have with it is that the elite roll is independent of the IDPD rank roll. What that means is that when only grunts are meant to show up, the game will still throw elite grunts at you as early as 1-1. Internally, the game increments the "heat level" at which point the game will start to throw more and more challenging units at the player.
Ammo tends to be an issue during the Mom fight unless you took a wrench, but once you can get past that the rest of the game tends to go pretty smoothly.
But on a serious note: there is a little purple line on the silhouette of the Lightning Hammer.
I like to play with music while also having the in-game sounds be there (so I don't get surprised by something that normally warns me with an audio cue), but there is no real sweet spot. I keep it at 3%, which is barely audible, while 4% is significantly louder, which is somewhat ridiculous.

There aren't exactly very many of those left in the game. You can pretty much count those on one hand now.
There is also the salamander sound effect that persists as well sometimes.
Not exactly a bug, but I still think it's worth noting.
Oh yeah, my stat menu still crashes.
