All provinces in the north italian region (so everything except napels) has to be in the HRE. So a member state has to conquer venice, urbinio and the papal state and add those provinces to the HRE.
If 1or more provinces isn't in the HRE in 1490, italy will leave the HRE.

It's more of a need to 'cause' them to stay as they leave unless certain things are done...

Yeah, what Shoki said.

I've played as Mantua (now Italy) since I got the new DLC and i'm fairly certain it was event-driven. At some point I just got an event asking if I wanted to leave the HRE. It was at the end of the 15th century roughly.
Unless the conditions outlined by shoki above are fulfilled, you'll get an option to leave or stay. Leaving the HRE causes a major relations hit with the Emperor, while staying will empty out your prestige.

In my Tuscany game, I was conflicted because I didn't want to be left vulnerable to encroachment by Spain or France. Every AI in Italy chose to leave, though, so I went with it.

Do the provences HAVE to be added to the HRE? Or do they just have to be owned by a member state?

I ask because I have several of those provences as I am working towards this goal, but I haven't cored them yet (Rome costs so much admin!) so I can't add them to HRE yet.

they have to be added, it annoymed me a lot. I took the last one i needed (rome) in 1489 then the even fired before i could core it.

All the HRE-members gets a choice to stay or leave, atleast i got that as savoy. I dont know if the choice comes no matter what though, since I only played in italt once since exp and in that game genoa of all countries conquered all venice land (yes including capital), they did not add the provinces to the hre though

But what the AI does? All leave, or it depends on alliances, relations etc?

Ok so I am now in control of the provinces that once belonged to Venice and Papal States. It is 1488 and I haven't received a message that I have succeeded. I did also core and add each of the provinces to the HRE. Do I have to wait until 1490 to receive a completion message?

Also, what do I do about controlling rome and having the constant -1 diplo rep because of the "Occupation of Rome" modifier? Maybe, after 1490 I can give it back to Papal States?

In theory it should free you to go for Hungary. Also more states in HRE means more bonuses for Emperor...

Dunno, if you get a Diplomat to get rif of all that bad relations, and go diplomacy group, I believe it´s worth trying.

To get more IA/month :D

Maybe, but for roleplaying purposes...

Same with Statute of Restrain of Appeals. Some stuff you don´t do to min-max, but to roleplay.

Also I played too little CS, but at least in previous patch North Italy seldom went Reformed, what exactly changed? Or is it bad luck from having center of reformation nearby?

If you look at the link I provided, the Northern Italy region is a whole lot more than Venice and the Papal State.

Since you get +1 IA for adding a province to the Empire, imo, letting them leave, conquering them, adding their provinces back and *then* releasing them, imo, would be much more efficient than just keeping them in.

Are you saying here that the EMPEROR must control all of these northern italy provinces, or they just need to be in the HRE some how?

My goal was to keep more HRE countries so I get a constant tick of positive IA, and to break up any of the larger countries so that I have more IA again. Also keeping Savoy and Switzerland and Milan in HRE keeps them from allying ith France or whoever against me.

Question: If I were to pull back out of northern italy and give the papal states back rome, would the rest of northern italy now leave the HRE? (It is past 1492 now)
