Also can you please try to set graphics to low and check if it's crash again or not?
I'm asking about that because there was few playes with HD3XXX HD 4XXX who got crashes due to GPU lockup.
If I get a chance I'll try attaching a debugger to find out which module is dying.

As I guess crashes on AMD graphics cards usually related to WDDM driver timeout caused by GPU lockup.
You can also get GPU-Z util and check your GPU temperature when crash happen:

For some player lockup caused by overheating and downclocking usually helps here.
I've created a WinDBG crash dump that hopefully you can use with the private symbols you have to debug the issue further. I've uploaded everything to my website here..
