darkseer : That's not the only place where you can get the achievement. There are other places, such as the cave in the Troll Claw Woods area.
Did it unlock immediately? I've chosen Glint, but when does Thrix come and take Glint's soul or anything like that? It didn't unlock for me when Glint left the party end-game.
This one took me multiple tries. First requirement is to free the good druids and accept their task. Only then one can side with Ferrusk and talk about the good druids. Second requirement apparently was to not simply kill the good druids, but first talk to them once more and start an attack that way.
What could "Fell to attackers outside Baldur's Gate" mean? Perhaps the attack on the coalition's camp?
hard way - yeah its any non optimal choice, theres a few ways to get it
chose poorly - unlocked right after i chose wrong...poor glint
dark ally - think the chat option to trigger it is something along the line of "what does your tree tell you now"
grim deeds - im guessing from the wording that "fell to attackers" implies being knocked out by the shadow thieves for the last cinematic as any other type of dead just ends the game
I've also not had to escape through the sewers in my previous playthrough. The sewers and the encounter of Corwin and Duncan was new for me.
i think if you have a max rep and you give 4 good examples to the duke during your trial a guard smuggles you out, less than max rep and a thief shows up and sends you through the sewers

I still can't figure it out how unlock "All the enemies" and "All the allies".
For me not either. No matter which NPC I sacrifice, there is no achievement not even if reaching the end of the game. I've reloaded a dozen times.
There is a comment in beamdog's forum. As enemies you need to offend Rigah & Julann, Strunk and Munsch so they fight against you at Dragonspear Castle. As allies, you can recruit Slug and Murs from the ogre bandit camp and "The Vanished" golem to fight for you at the castle.
Haven't tried those options yet, because even if playing on story mode, it may be necessary to start a fresh game as not to have something corrupted in old savegame state data.
I started new game to get achievements from companions (though Khalid quest need another playthrough from the Bridgefort, so I made backup save now and continue with Minsc & Dynaheir, Goblin, Neera and Glint).
I don't remember who is Slug. Is he that ogre caged beneath Dragonspear Castle? And I don't remember Munsh either.
"Munsch" should be Delgar Munsch in the crusader camp at Bridgefort.
In case of "All the enemies" - I surrendered Bridgefort to Barghest, so there was no battle and Delgar Munsch left the area living and breathing. I talked with him in the camp. Then I met Rigah and Julann with Minsc and Dynaheir in my party and I tought if I won't show them evidence about crusader's corruption they will join the crusade during siege. After that I tolked with Strunk and left him and his water elementals.
During siege Munsch, Rigah and Julann were absent. Only Strunk appeared with his elementals (he was the only one which gave journal status about killing him or leave him living, besides of "Wychlaran and the berserker" quest which I didn't do because I didn't want to Rigah and Julann leave the crusade).
So I still don't have "All the enemies", "All the allies", "Proud of yourself", "It wasn't me!" and ofcourse those which are bugged "You chose... poorly" and "Grim deeds to come".
I will get "It wasn't me!" soon in normal playthrough (not speedrun) with my paladin, but I can't figure out how get rest of the achievements. And still - my mainchar with 18 charisma didn't triggered "Proud of yourself" and I don't know why. I speaked to Oloneiros in the camp near Bridgefort, after resquing crusaders patrol, and after revealed my name she just turned hostile with entire camp.
