from memory I think you have to be within a 'known' area, somewhere you have travelled to and the new landmark has to be close to that - but I may have misremembered. DarkStarSword, who frequents this topic should be able to put you right - or maybe look up Miasmata cartography on Google. This might help http://miasmata.wikia.com/wiki/Cartography

That's only the first part of cartography - "triangulating your location and surveying the local area". The surveying part is why any nearby landmarks become known when doing this, and that is also why the weather sometimes seems to suddenly change when doing this - surveying takes quite a bit of time, sometimes enough for the weather to change or night to fall.

You can also make distant landmarks known without having to travel to them - this is how you find the location of landmarks on islands that you can't swim to, but is also useful for other landmarks. First you need to triangulate your location from two known landmarks, then you can click on any other (distant) unknown landmarks you can see from there - these will have a + icon when you mouse over them, but only after you have triangulated your own location.

You then travel to a second spot where you can also see the unknown landmark. This has to be more than a few degrees away from the first spot, i.e. travelling straight towards or away from the unknown landmark won't work - you have to travel in a different direction so that you are forming two corners of a triangle with your position and the third corner being the landmark.

Then repeat that process - triangulate your location from two known landmarks, then click the unknown landmark again (it will have a + icon if you have moved far enough - if it still has a ? icon you either haven't moved far enough to form a triangle, or haven't triangulated your own location yet) and it will then become known (and will now have a simple triangle icon when moving the mouse over it) and can be used to help triangulate your location from then on.

P.S. The mental clarity tonic will instantly reveal your location on the map, which is useful if you have become completely lost and cannot see any known landmarks nearby, but it will not survey the local area and cannot be used to triangulate distant landmarks - if you wish to do that you still need to triangulate your location as though you hadn't taken then tonic. Opening the map while standing right next to a known landmark will also immediately reveal your location, but just like the tonic it will not survey the local area and will not allow you to triangulate distant landmarks - once again you will have to accurately triangulate your location first to allow that.

P.P.S. If you're an achievement hunter, the head statue achievement will only work if you triangulate the final statue - if you reveal it by surveying it will not unlock and you will need to use miaschiev to fix your save file.
