Examples: The item that applies burn to your special won't apply a second burn until the first wears off. If two players have a poison item, they can both be applied at once. You can have two different sources of the Smite buff at the same time.
Player buffs from skills can refresh themselves, like Warcry.
Debuffs that only do damage at the point of wearing off do not stack and do not refresh (Burn and Snare). I also haven't seen alternate versions of them even from different sources. If you are playing Dancer and get the special applies burn gem do not take the special applies burn item. You'll gain nothing.
Edit: Hmmm, seems I might be wrong on this. Maybe it was changed after I last tested it. I definitely remember being disappointed after trying to stack burn on special from different sources.
The person above is wrong on stacking. As long as its not the exact same source every debuff stacks.
Item that burns on special use + skill upgrade that burns on special use = 2 burn stacks.
Skill upgrade that burns on special use ONLY and you use it twice in a row = 1 burn stack, not even refreshed.
burn on primary + primary hits 3 times = 1 burn stack.
Honestly I feel like debuffs like curse (maybe that's the only one) should be refreshed per hit (i.e. shouldn't have to wait for it to time out). Yeah it's a buff, but I feel like it should be a thing.
