Still I think that being worried about dying and starting over is the first step towards failing the run. Also learning the whole game by heart is a must, but I guess you already know it, lol
This is the same mindset that helped me with other no-death runs in other games, basically just try to enjoy the game without worrying too much about dying, but still taking it nice and slow. The more times you fail, the more you can learn which points of the game or which enemies get you killed the most, and prepare in advance... and don't be afraid of cheesing stuff, lol, unless it's against the rules of the run of course.

This helped me with the less noble problem of repeatedly running at bosses with no strategy at all in my first ds1 playthrough. Should apply to ds2 though, that looks pretty deterministic.
Way less savy in the hardcore in this than you and others of course. Got years of hardcore Might and Magic Run Experience though.
Trying to map problem areas on another character, remembering spots that give me trouble in previous play through (is probably the biggest mental block). In these mapping sessions making of check list of every way I get killed especially in stamina damage ratio. Practicing and muscle memory is a lot more prominent in Dark Souls. I mean we’ve all beaten this game multiple times and “know it” like the back of our hands (at the same time always learning something new ) so just reminding that to ourselves like a mantra helps.
As been mentioned saying your plans out loud helps and turning them again into a mantra and playing it into a larger joke may help lighten the soul a bit.
The bosses weaknesses in DS2 to poison is ridiculous. Less than 14 poison daggers and the smelter demons out for the count. So that’s an arsenal up your sleeve to consider in your planning.
Helping slow that rushing thing down is to pace yourself with a bow. Slows the game down immensely and opens the window to avoid many ambush tricky spots may help.
And maybe practicing and mapping out around that blasted Iron King’s hole can stem that fear off too.
So yeah just do your thing, you know the game way more than I do. It’s about the no death run not the rush this time around. I usually preform better when I don’t try to avoid dying (I mean thinking to myself this is Dark Souls and I’ll die rather than panicking over getting nicked or in a proximity angle of the Aegises back swing).
Soothing music of your choice helps, or no music at all. In Dark Sousl 3 runs I tend to shut off the dialogue for the bosses as well.
